Apply In Person
The first option is to visit your local SSA office and request a new card. For residents of Florida and several other states, you may be able to apply online, if you meet these guidelines: if you are 18 years of age or older, have a United States address, are not requesting a name change on your card, and have a state-issued driver’s license or identification card.
